PIERCE TRANSIT
MEDICAL SERVICES FOR APPLICANT AND EMPLOYEE PHYSICALS

INTRODUCTION AND OBJECTIVE
Pierce Transit has a continuing need for professional medical services for applicant physical examinations and employee physical examinations. Additionally, Pierce Transit has occasional needs for expert medical opinions on matters of occupational health or equal employment policy, supervisory concerns, employee problems, grievances, and arbitrations. All of these activities require that Pierce Transit have access to medical expertise.

Pierce Transit is soliciting proposals from qualified health care providers to provide physical examinations of job applicants, employees returning from extended absences, and other employees as requested by Pierce Transit.

Pierce Transit is a Washington State Municipal Corporation organized under RCW36-57A, with 945 employees and has a responsibility for providing fixed route and elderly and disabled public transportation services in urban Pierce County, from Tacoma to Olympia and Seattle. The Agency's administrative offices are located at 3701 96th Street SW, PO Box 99070, Tacoma, WA 98499-0070.

SCOPE OF WORK
The scope of the services to be provided by the selected medical facility include the following:

  1. As required conduct on-site job evaluations of Pierce Transit jobs, develop job standards that include physical and health demands in accordance with ADA regulations. Review and confirm job standards with Pierce Transit management. Update and review job standards biennially.
  2. Perform physical examination services at the request of Pierce Transit's Human Resources Department, provide a complete written report in standard Pierce Transit format and recommendations as to the applicant's or employee's ability to perform the duties of the job and any applicable restrictions.
  3. Provide a recommendation for an applicant’s ability to perform the essential duties of a job based on review of physical capabilities exam or assessment exercise, conducted either by the Contractor or an independent physical therapist.
  4. Request and evaluate laboratory tests sufficient to render a medical decision regarding the applicant's or employee's health as it relates to the position for which he/she is examined.
  5. The physician is expected to conduct pre-employment and return-to-work physical examinations and render a decision as to the applicant's ability to fully perform the essential functions of the job. DOT and non-DOT physical examinations shall at the minimum, contain the following:
    1. Completion of a comprehensive health and occupational history questionnaire, including inquiries into applicant's on-and-off-job injury history as related to the job duties.
    2. Measurement of job related work activities such as lifting, pushing, turning, etc., against a load, according to proper physical and occupational therapy procedures.
    3. A complete head to toe examination by a licensed physician.
    4. Urine sample collection according to federal DOT regulations, 49 CFR Part 40; dipstick evaluation test to include a minimum of specific gravity, PH, protein, and sugar; (conveyance of sample with proper protection of chain of custody to Pierce Transit's contracted laboratory for drug screening).
    5. X rays, if deemed necessary by the physician and approved by the Agency's Human Resources Department.
    6. A routine hearing test for drivers and maintenance employees to establish that employee’s baseline hearing levels for the Agency’s Hearing Conservation Program (WAC 296-62-09015 through 296-62-09055).
    7. Other tests as deemed necessary by the physician and approved by the Agency's Human Resources Department.
  6. In accordance with applicable federal drug testing regulations, the Contractor shall prepare, and, if necessary, store urine specimens for drug testing by Pierce Transit's contracted certified laboratory. Sample collection, chain of custody, and security shall follow procedures established by 49 CFR Part 40.
  7. Conduct medical evaluations of employees required to use respirators (to be identified by Pierce Transit) including a review of the required questionnaire in accordance with WAC 296-62-07150 through 296-62-07156.
  8. Physical examination of employees for other causes will be conducted as needed on a case-by-case basis as requested by Pierce Transit and for the conditions specified. Medical examinations, including non-DOT drug and alcohol testing, may be required to determine an employee's physical and/or mental fitness for duty. The results should include an evaluation of the issue and recommended action for Pierce Transit.
  9. Expert testimony, including explanation of tests and examination results, as needed.
  10. Report on the results of each test to the Agency's Human Resources Department by telephone within twenty-four (24) hours of the physical. Provide complete written, quantified test results to the Agency's Human Resources Department within three (3) days of the examination in a format acceptable to Pierce Transit.
  11. Maintain all necessary records of services provided and appropriate procedures to assure confidentiality of records and reporting.
  12. Provide timely, detailed invoices of services provided.
The provider shall have the ability to regularly schedule physical examinations within five (5) business days of Pierce Transit's request and on shorter notice when necessary.

QUALIFICATIONS
The successful medical provider shall have at a minimum the following qualifications:

  • License to do business in the State of Washington.
  • Proper professional licensure of all medical personnel who will be providing direct services to individuals referred by Pierce Transit.
  • At least three (3) years experience performing the types of services specified.
  • Demonstrated ability to meet the response times specified.
  • Demonstrated ability to provide the specified services, reporting, record keeping, and confidentiality on an ongoing basis.

TERM OF CONTRACT
The contract resulting from this request for proposals shall be for a three-year period with an option to renew the contract for two additional one-year periods upon mutual agreement by both parties.
